FS#644 TOC links to the same anchor when the same headings is present on the page

When you have a page with a structure like
===== Heading 1 =====
==== Summary ====
==== BlaBla ====
===== Heading 2 =====
==== Summary ====
==== BlaBla ====
The generated TOC have the goog structure displayed, but the links for "Summaries" point all on the first on. (the 2 anchor exists but have the same name)
Maybe using a sequence if an anchor already exists when creating a new one could be done.
