On a brand new installation, I put the following on the main page
{{:blyderivercanyon.jpg|The Blyde river canyon}}
An image is displayed.
[[test page]]
A link is produced.
[[test page|a test page]]
A test-substituted link is produced.
[[test page|{{:blyderivercanyon.jpg|The Blyde river canyon}}]]
On first load, an error is thrown (see screenshot). On subsequent loads, the image is not displayed, only the link.
This wiki markup works perfectly on another server running Dokuwikistick-2009-02-14 and mApache.

I am having the same or a similar problem. The DokuWiki is installed on a Windows server with IIS. Image files (png, jpg, and gif) are not being displayed but the link is shown. The files seem to be uploaded successfully, and I see them in the appropriate media namespace. I checked syntax, and it is the same as on another DokuWiki installation on a Linux server where images are displayed correctly.

I reverted PHP to version 5.2.10 and the problem was resolved. Suggest marking Dokuwiki as incompatible on IIS with PHP 5.3.0 until the cause of the problem is resolved.

Same thing here. As I have no IIS but Apache, it seems to be an issue of PHP 5.3.0 with DokuWiki-2009-02-14b.
Forgot to mention some system details:
* DokuWiki-2009-02-14b
* PHP Version 5.3.0
* Apache/2.2.9 (Debian) proxy_html/3.0.0 mod_ssl/2.2.9 OpenSSL/0.9.8g
(virtual?) server of an internet host company, I don't have admistrative permissions there
* Browser: Firefox 3.5, IE7
